Does anyone know how to obtain this paper?

That might be a tough nut to crack, Tony. Hopefully someone on the forum has it. I tried searching on the SPIE site and couldn't find it.
Too bad we're not at the University of Michigan. They have practically every SPIE volume on holography ever published, just sitting on the shelves and available to the public, with copiers nearby.

Just send an eMail to R D Rallison, I did it 5 years ago and he was very kind to reply.
Sure if you ask with holographical arguments, he can help you.
